IMA Urgent Care Locations

IMA Urgent Care Centers offer an alternative to the hospital emergency room for minor emergencies, as well as acute and chronic illnesses. No appointment is necessary at these offices.  If you are an IMA Primary Care Patient, your basic information is available via our computer link.

  • Health Insurance Information
  • Depending on your insurance coverage, a method of payment at the time of your visit

Personal Health History Information

Please have to the best of your knowledge the following information:

  • A list of current medications
  • A list of past surgeries
  • A list of any allergies
  • Any information about your current illness including symptoms
