Troubleshooting Patient Portal
Forgot Username
Step 1: Go to www.imamd.com
Step 2: In top right corner, click on “Patient Portal” which brings you to the sign in page
Step 3: Click “Forgot Username”
Step 4: Enter requested information – First Name, Last Name, Date Of Birth, Zip Code
Step 5: Click “I am not a Robot”
Step 6: Click “Submit”
Step 6: Test for “I Am Not A Robot” will appear. You must take that.
Step 7: Check your email. You will receive an email with your username
Step 8: Make sure you check your junk/SPAM email
Step 9: Click on the link in the email to sign in
